Physical Therapist- Full-time Denver

Are you a dedicated Physical Therapist looking to make a real difference in the comfort of patients' homes? We're seeking a compassionate and skilled PT to join our home health team in the vibrant city of Lancaster. This full-time position offers the unique opportunity to provide personalized care to our diverse client base, helping them achieve their health and mobility goals.

What You'll Do:

Responsibilities:  

  • Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
  • Performs patient evaluations in accordance with agency policy and procedure. Selects appropriate diagnostic material to assess all aspects of patient care.
  • Informs the physician of the results of the assessment and recommended clinical interventions prior to initiating care.
  • Develops and documents appropriate treatment plans for patients including selecting appropriate intervention approaches, strategies, materials and establishing treatment priorities.
  • Provides an organized and efficient patient treatment which includes advising, educating and counseling the patient.
  • Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.
  • Provides appropriate treatment to patients according to patient needs and within the parameters set by the agency and payer.
  • Determines appropriate goals, which are objective and measurable in nature, in collaboration with team members and patients.
  • Meets agency productivity standards and uses work hours efficiently.

What We’re Looking For:

  • Minimum nine months experience as a physical therapist
  • Preferred community/home health experience

Required Certifications and Licensures:

    • Master’s degree in physical therapy approved by CAPTE accredited physical therapist education program
    • Currently licensed to practice as a physical therapist within the state of agency operation and compliant with accepted professional standards and practices
    • Must possess and maintain valid CPR certification while employed in a clinical role
    • Must be a licensed driver who can travel to all business locations
