What You’ll Do
-
Provide primary care services for pediatric and adolescent patients, including health screenings, diagnosis, treatment, and preventive care.

Collaborate with physicians and the care team to deliver comprehensive, coordinated services.

Refer patients to specialists when appropriate and follow up on care.

Participate in quality improvement, peer reviews, and department initiatives.

Use and maintain accurate documentation through our Electronic Health Record (EHR) system.

Coordinate care with other CHS departments and community partners to support holistic health outcomes.
What We’re Looking For
Required:
-
Graduate of an accredited nursing school and approved Pediatric Nurse Practitioner program.

Active (or eligible) APRN licensure in the State of Connecticut.

Eligible for DEA and CT controlled substance registration.

Strong knowledge of pediatric practice, clinical decision-making, and patient assessment.

Excellent communication and documentation skills.

Cultural competence and a passion for serving diverse communities.

Comfort with electronic systems and EHR platforms.
Preferred:
-
1–2 years of experience in a pediatric or primary care setting.

Spanish-speaking ability.
