RN Home Health
Responsibilities:
- Open cases with assessment and develop a plan of care
- Provide appropriate treatment to achieve positive patient outcomes
- Supervise LPNs and HHAs
- Educate patients and families to improve outcomes and ongoing care
- Work closely with other team members and administrative staff
- Complete all documentation timely and accurately
Requirements:
- A Registered Professional Nurse with current License
- 1+ year nursing experience
- Home Care nursing experience a plus
- OASIS experience a plus
- Med-surge experience a plus
- Wound care and PleurX catheter experience desired
- Ability to travel between patients' homes
